This course is ideal for front line managers who are required to participate in incident investigations.

This 3-day, workshop style course examines a broad range of topics including safety, health, environmental, quality, reliability and production-process incidents.  It teaches a systematic approach for investigation, analysis and implementation of correction actions to improve HSE performance, prevent injuries, property damage and financial losses.

On completion of the course participants will have an understanding of:

• Identifying underlying causes of incidents and near misses
• Identifying key objectives and issues in implementing an investigation
• Identifying potential sources of evidence
• Legal issues
• Systematic methods of conducting interviews and effective questioning to gain key witness information
• Applying appropriate techniques to identify the sequence of events that result in incidents
• Applying tools such as fault tree and flow chart analysis to assist in  the investigative process
• Using techniques, models and other methods of analysis to help identify immediate and root causes
• Critical information needed to comply with HSE regulations
• Developing a corrective action program, control procedures and follow up
• Evaluating existing Incident Investigations processes through auditing
